Submitting Feedback

After clicking "Answer the questions" we are ready to submit feedback.



You should be offered another tour here. 

Download the Report


Click the pink link to download the report.

Select Approve/Disapprove

Select from the multiple choice question your response.
Click "Next page" to continue.

If you disapproved you will get an extra page with a text box so you can enter your reasons.


After entering your reasons and clicking submit or if you selected approve initially you will be taken here...

Clicking continue will take you to the next report.
